The added value of an integrated DHIS2 and RBF data system"},"content":{"rendered":"\n<p class=\"graf graf--p graf-after--h4 wp-block-paragraph\" id=\"64f5\">A large number of countries in which we work have shifted their HMIS (health management information system) onto <a class=\"markup--anchor markup--p-anchor\" href=\"http:\/\/dhis2.org\/\" target=\"_blank\" rel=\"nofollow noopener\">DHIS2<\/a>\u200a\u2014\u200aa popular open source software tool developed by the University of Oslo. Often, the data platform also becomes the country\u2019s de-facto health system data warehouse.<\/p>\n\n\n\n<p class=\"graf graf--p graf-after--p wp-block-paragraph\" id=\"1b42\">While using DHIS2 has many clear advantages, one major benefit for countries is that it allows for the integration of multiple vertical data systems into a single data platform. In other words, data collected through one system becomes available to all through a centralized hub. For example, HIV, TB (tuberculosis), NCD (non-communicable disease) specific data systems can now be merged into the national DHIS2.<\/p>\n\n\n\n<p class=\"graf graf--p graf-after--p wp-block-paragraph\" id=\"2bdb\">However, DHIS2 does not solve the issue of data reliability. Meaning that the technology alone cannot solve issues related to the poor quality of reported data, irregular reporting and perverse incentives. But results-based-financing (RBF) can.<\/p>\n\n\n\n<p class=\"graf graf--p graf-after--p wp-block-paragraph\" id=\"aa4d\">RBF adds value to routine data systems in three concrete ways:<\/p>\n\n\n\n<ul class=\"postList wp-block-list\"><li id=\"2ec7\" class=\"graf graf--li graf-after--p\">RBF creates an incentive for better management of data at both health care facility and district levels. Why? Because once money is tied to data, stakeholders tend to care more about data. Linking health facility financing to data leads to better data completeness and reporting.<\/li><li id=\"37e8\" class=\"graf graf--li graf-after--li\">Data verification mechanisms have been developed in most countries using RBF systems. External teams visit a given facility on a regular basis and triangulate the declared data to check its accuracy. In several countries, the difference between declared data and verified data has gradually been reduced over time, showing an overall improvement on how health care facilities manage and report data.<\/li><\/ul>\n\n\n\n<figure class=\"wp-block-image graf graf--figure graf-after--li\" id=\"0de6\"><img decoding=\"async\" data-src=\"https:\/\/www.bluesquarehub.com\/wp-content\/uploads\/2016\/09\/156a5-1e613huzi2gj1jchvfgztyw.png\" alt=\"\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" class=\"lazyload\" \/><\/figure>\n\n\n\n<ul class=\"postList wp-block-list\"><li id=\"1fa8\" class=\"graf graf--li graf-after--figure\">Finally, in most countries that implement RBF routine, quality of care evaluations are being conducted, often on a quarterly basis. To do this, a structural quality checklist is frequently used as a proxy for quality of care measurement. These checklists focus on service availability and readiness, and are a simpler version of the typical <a class=\"markup--anchor markup--li-anchor\" href=\"http:\/\/www.who.int\/healthinfo\/systems\/sara_indicators_questionnaire\/en\/\" target=\"_blank\" rel=\"nofollow noopener\">SARA questionnaire<\/a>. BlueSquare currently helps seven countries digitize this structural quality measurement evaluation. In shifting these routine health care surveys carried out as part of RBF on mobile devices, countries can start to collect 100\u2013200 data elements per health care facility per quarter. In that sense, RBF brings a new \u201cexternally verified service availability and readiness\u201d data set to routine HMIS systems.<\/li><\/ul>\n\n\n\n<figure class=\"wp-block-image graf graf--figure graf-after--li\" id=\"e969\"><img decoding=\"async\" data-src=\"https:\/\/www.bluesquarehub.com\/wp-content\/uploads\/2016\/09\/1954b-1jhsroyeezs9awd77d9ldyq.png\" alt=\"\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" class=\"lazyload\" \/><\/figure>\n\n\n\n<p class=\"graf graf--p graf-after--figure wp-block-paragraph\" id=\"a4db\">To recap, the integration of RBF and DHIS2 platforms is of high value to HMIS data. It brings better, verified data, and a new large routine service availability and readiness data set.<\/p>\n\n\n\n<p class=\"graf graf--p graf-after--p wp-block-paragraph\" id=\"f63f\">As a result, several countries, including Benin, Democratic Republic of Congo, and Kenya are asking us to connect and integrate RBF and DHIS2 platforms. This interoperability requires an alignment of the health system hierarchy (the org unit hierarchy in DHIS2) and an alignment of the RBF \u201cindicators\u201d with the DHIS2 \u201cdata elements\u201d.<\/p>\n\n\n\n<p class=\"graf graf--p graf-after--p wp-block-paragraph\" id=\"8077\">Once this is in place, interoperability has several advantages:<\/p>\n\n\n\n<p class=\"graf graf--p graf-after--p wp-block-paragraph\" id=\"f7ff\">First, declared data from health care providers is collected only once through the HMIS. Secondly, the national DHIS2 platform is used for RBF monitoring and evaluation (M&amp;E). Third, any \u201cRBF\u201d data (verified, quality of care, patient feedback) is available for any other program in the DHIS2 platform.<\/p>\n\n\n\n<figure id=\"18c8\" class=\"graf graf--figure graf-after--p\">\n<div class=\"aspectRatioPlaceholder is-locked\">\n<div class=\"progressiveMedia js-progressiveMedia graf-image is-canvasLoaded is-imageLoaded\">\n<\/div><\/div><\/figure>\n\n\n\n<figure class=\"wp-block-image alignnone progressiveMedia-image js-progressiveMedia-image\"><img decoding=\"async\" data-src=\"https:\/\/www.bluesquarehub.com\/wp-content\/uploads\/2016\/09\/ba0fe-1l-si2stgqg9cc5bbtmewng.png\" alt=\"\" src=\"data:image\/svg+xml;base64,PHN2ZyB3aWR0aD0iMSIgaGVpZ2h0PSIxIiB4bWxucz0iaHR0cDovL3d3dy53My5vcmcvMjAwMC9zdmciPjwvc3ZnPg==\" class=\"lazyload\" \/><figcaption>RBF quality of care monitoring in DHIS2\/ROC<\/figcaption><\/figure>\n\n\n\n<p class=\"graf graf--p graf-after--figure wp-block-paragraph\" id=\"5c81\">But, the biggest advantage is yet to come.
